Parents/Guardians must provide the following documents:

Student’s birth certificate (State copy is preferred)

Student’s social security card

Student’s immunization record

Enrolling parent’s/guardian’s driver’s license

Court orders proving guardianship, if applicable

Proof of Residency:

a. Current electric or water bill with physical address listed

b. If renting or residing with someone, notarized forms provided

by school along with current proof of residency described above

There is a notary at LVISD Central Office.

Pre-kindergarten Requirements

Since the Pre-kindergarten program at La Vernia Primary School is federally funded, students MUST qualify by satisfying the following:

· Age 4 as of September 1, 2006

And one of the following:

· Be considered an English as a Second Language Learner

(another language other than English is spoken in the home)

· Qualify for Free/Reduced meals (determined by LV’s Child Nutrition Director’s evaluation of Free/Reduced application based on federal guidelines

· Be considered homeless by the McKinney-Vento Homeless Education Assistance

Improvement Act, 42 U.S.C.11435

· Children of active duty members of the armed forces including the child of a member who was hurt or killed on active duty.
