IMMUNIZATION
REQUIREMENTS
State law and LVISD policy require
medically validated records of immunizations to be on file for all
students. ANY STUDENT WHOSE IMMUNIZATION RECORDS ARE NOT COMPLETE
WILL BE SUBJECT TO EXCLUSION. ALL STUDENTS ARE REQUIRED TO BE FULLY
VACCINATED AGAINST THE SPECIFIED DISEASES. However, a student may be
provisionally admitted if the student has begun the required immunizations
and continues to receive the necessary immunizations as rapidly as
medically feasible. In addition, an in-state transfer student who is
fully immunized or has begun the required immunizations has 30 days from
the date of enrollment to produce the required documentation.
DIPHTHERIA-PERTUSSIS-
Five doses of a diphtheria-tetanus-pertussis containing vaccine, one
TETANUS (DTP/DTaP/DT) of
which must have been received on or after the fourth birthday;
however, 4 doses meet the requirement if
the 4th dose was given on
or after the 4th birthday.
Ages 7 years or older
(pertussis vaccine is not required) Three doses including one dose on or
after the fourth birthday
Tdap/Td/DTP/DTaP

Exemptions:
The law allows (a)
physicians to write a statement that the vaccine(s) required would be
medically harmful or injurious to the health and well-being of the
child, and (b) parents/guardians to choose an exemption from immunization
requirements for reasons of conscience, including a religious belief.
The law does not allow parents/guardians to elect an exemption simply
because of inconvenience (a record is lost or incomplete and it is too much
trouble to go to physician or clinic to correct the problem).
For children needing
medical exemptions, a written statement by the physician should be
submitted to the school nurse.

Schools maintain an
up-to-date list of students with exemptions, so they can be excluded from
attending school if an outbreak occurs.
Documentation:
Since many types of personal
immunization records are in use, any document will be acceptable provided a
physician or public health personnel have validated it. The month, day, and
year that the vaccination was received must be recorded on all school immunization
records created or updated after September 1, 1991.
