Registration forms with current, accurate information are required for each student.  This form is essential in providing care to your child while at school.  Current work and home telephone numbers for parents and other adults who are authorized to pick up your child must be listed. CHANGES IN WORK OR HOME TELEPHONE NUMBERS MUST BE TURNED IN TO THE OFFICE.  Cell phone numbers and pagers should be listed.

Students will not be released to anyone not listed on the form.

EMERGENCY CARE- If a student is injured or becomes seriously ill at school and the parent cannot be reached or does not respond, the child will be transported by EMS .  La Vernia Independent School District does not assume any financial responsibility but will provide or arrange emergency care.

Parents are required to complete a Student Medical Status Update on their child at the beginning of each year.  Should your child have a special health need requiring special procedures, P.E. limitations, and/or emergency care and/or actions while at school, the school nurse will be in contact with you and your physician for documentation.  Special health needs and information will then be shared with appropriate school personnel to care for your child.
